Population Health
A population health approach uses quantitative and qualitative evidence on the determinents of health to identify priorities, dispartities and strategies to improve the health of a population. Action is directed at the health of an entire population, or sub-population, rather than individuals. Priority population groups are identified as subgroups of the general population who have a reduced quality of health or access to health services. The outcomes of a population health approach, therefore, extend beyond improved population health to include a sustainable and integrated health system which is a shared responsibility among health sectors and other groups.
